The EDS Individual Plan offers comprehensive dental benefits at a low monthly cost. You can choose your general dentist from one of Arizona’s largest prepaid networks.

Enrollment is fast and easy. Simply complete the Enrollment Application and Agreement and the Payment Method Form. EDS offer two payment options. You may pay the entire yearly premium for additional savings or authorize a monthly bank deduction. The choice is yours. After your effective date, you can begin to enjoy the benefits of your plan.

Save 20% to 60% on Preventive Procedures, Including Exams, X-rays and Cleanings.

Save 30% to 40% Basic & Major Restorations Fillings, Crowns, Dentures available at deep savings.

Save 20% on Orthodontics including Braces for both children and adults.

Average annual savings of $1,200 per family on dental work.

Over 49,000 participating providers nationwide and includes specialties; Endodontics, Oral Surgery, Orthodontics, Pedodontics, Periodontics, and Prosthodontics. All dentists must meet highly selective credentialing standards based on education, background, license standind and other requirements. Members may visit any participating dentist on the plan and change providers at any time.
